Remote Physiologic Monitoring (RPM)

Studies have proven that patients engaged in clear and frequent communication with their Physician or healthcare professionals are more likely to follow care recommendations, appropriately monitor their health condition, and alert providers to relevant medical issues to avoid critical healthcare episodes. In the past few years, many leading healthcare systems and provider practices have dedicated resources and staff to offer and provide Remote Physiologic Monitoring (RPM) services to patients to focus patient engagement for their most at-risk chronic disease patient populations.

RPM technology utilizes various devices that monitor glucose levels, sleep patterns, heart rate, blood pressure, and many other types of patient data. RPM allows patients to be involved in their own care by giving them access to their health data in real-time.

Medicare provides reimbursement for RPM services under the same conditions as in-person physicians’ services — there are no additional requirements. Also, RPM services are not expressly required to use interactive audio and video.

Providing Remote Physiologic Monitoring Services with WSP

Medicare requires the documentation of at least one chronic condition (Principal Care Management – PCM), or two or more chronic conditions (Chronic Care Management – CCM), for patients to qualify for RPM services. A disease-specific care plan must be implemented and a minimum of 30 minutes per month of non-face-to-face patient care contact and tasks associated with the care plan documented for monthly services reimbursement.
